The DDKY Partner Relief Fund has been established to provide assistance for Delta Dental of Kentucky appointed Agents and/or Agencies, and current Employer Groups.

If you are a Delta Dental of Kentucky appointed Agent and/or Agency, or a current Employer in need of immediate relief funds, please complete the Relief Request Form.

You can expect to hear back from a Delta Dental of Kentucky Disaster Relief Team Member within 48 hours regarding the approval of your relief request.

A check will be prepared and mailed directly from the Delta Dental of Kentucky office located in Louisville, Ky.
